Adnexal mature cystic teratoma (dermoid cyst)
  • A 30-year-old female patient presented with lower abdominal pain.
  • Non-contrast CT:
    • A well-circumscribed left adnexal lesion is seen with internal mixed inhomogeneous fatty attenuation elements, soft tissue densities and dense tooth-like structure. It measures about 4.9 x 4.8 cm at maximum dimensions.
  • The diagnosis of mature cystic teratoma at CT is straightforward because fat attenuation within a cyst, with or without calcification in the wall, is diagnostic for mature cystic teratoma.
  • Whenever the size exceeds 10 cm or soft tissue plugs and cauliflower appearance with irregular borders are seen, malignant transformation is considered.
